The window to submit claims for copay reimbursement is open from July 1st through August 31st.  

IMPORTANT INFORMATION
• NO Excel spreadsheets or individual receipts will be accepted.
• All copay information listed on the PDF document MUST BE VISIBLE. Any information that is blacked out will be omitted and not calculated.
• In accordance with HIPAA regulations, dependents who are 18 years of age or older may be required to obtain their own summary.
• Physical Therapy, Occupational Therapy, and Counseling services may require submission of an Explanation of Benefits (EOB), as multiple dates of service may be billed under one claim.
• To ensure full reimbursement, each date of service must be included.
• Benefit Fund does not accept claims via fax or email.
• The S.O.A. Benefit Fund does not provide assistance with website access or passwords.
• Claims may be submitted ONE TIME ONLY. No additional adjustments will be made after the claim has been processed.
