The Express-Scripts/EMHP summaries were mailed out in February. If you have not received yours, please contact Express Scripts and request the 2024 Explanation of History Print Report.

The Fund advises that you wait until you receive all statements for all eligible dependents prior to submission.

You can only submit once.

If you are a Medicare Prime member, you Must call ESI for the 2024 Explanation of Benefits History Print or submit a pharmacy summary.

The submission deadline for all 2024 prescription claims is April 30, 2025.
