The preferred method is to call Anthem and ask for the 2024 Medical Copay summary for the family.

Anthem will email the report.

This report is more concise.

If you log into your account on the Anthem website and follow the instructions on the form, you should be able to print the report. **However, please note that the report may contain a large number of pages.
