The window to submit claims for prescription reimbursement is now open through April 30th.
- The EMHP / Express-Scripts 2025 Summary is the preferred report of acceptance. They should be mailed out by the end of February.
- ALL members who have Medicare, will NOT receive the EMHP Summary due to Medicare Regulations.
- If you choose the online submission method, refer to the 2025 Claim Form for precise directions and filter by patient name.
- Only One (1) form per family will be accepted. We encourage you to wait until summaries for all members and eligible dependents have been received before submission.
- Claims will be accepted & processed ONE TIME ONLY.
- NO Excel Spreadsheets will be accepted, including the “2025 Claims History” listed at the top of ESI website when logged in.
- Claim forms must be submitted with the Prescription Summary.
Medicare Members :
1. Contact Express Scripts (866) 340-8996 and request the "2025 Explanation of Benefits History" report. Advise them that the report cannot be in Excel format.
OR
2. Submit a Pharmacy Printout for Jan 1–Dec 31, 2025
